BMH To Conduct A Community Health Needs Assessment

Brattleboro Memorial Hospital is in the process of conducting a Community Health Needs Assessment as required by the Patient Protection and Affordable Care Act. The purpose of the survey is to assess the heath needs of our community.

On May 18th, 2012 a group of 21 key community leaders came together at BMH to participate in a focus group that identified key health needs for various population age groups, as well as barriers to addressing these gaps. The focus group participants made suggestions that they thought would improve the health of the various population age groups.

As the next phase of the community health needs assessment process, BMH invites community members as well as BMH/SVHSC employees to complete an on-line anonymous survey by clicking the picture below.

If you prefer to complete a paper copy of the survey, contact Wendy Cornwell, Director of Community Health and Hospital Education at 802-257-8325, and one will be provided to you.

Note that the survey will take approximately 10 minutes to complete and is completely anonymous. Your opinions are valuable to BMH and we greatly appreciate your time. Thank you.
