Behavioral Specialist Added to Blueprint Community Health Team
The Brattleboro Hospital Service Area of the Vermont Blueprint Community Health Team today announced the addition of Michelle Harmon, a Licensed Clinical Mental Health Counselor.
Harmon becomes the third member for this area’s Community Health Team, according to Vermont Blueprint for Health Project Manager Wendy Cornwell. Through a contract with Healthcare and Rehabilitation Services of Southern Vermont, Harmon will work collaboratively with patients at Windham Family Practice, Brattleboro Primary Care and the Support and Services at Home Program (SASH).
“Michelle brings a wealth of clinical experience with patients with substance abuse, dual diagnoses, and other mental and behavioral health issues,” says Cornwell, who is also the Director of Community Health and Hospital Education at Brattleboro Memorial Hospital. “A Behavioral Health Specialist uses motivational interviewing techniques, advocacy skills and knowledge of mental health and community resources to help patients overcome barriers and facilitate their implementation of healthy lifestyle changes. We’re excited to welcome Michelle to the team.”
