The Advance
Care Planning Initiative is a cooperative effort by
Brattleboro area community volunteers to provide public
education and facilitate community dialogue on making
decisions about end-of-life care. Among the members
of the Initiative are several nurses and nurse practitioners
from Brattleboro Memorial Hospital.
These people, who trained with other community members
about advance care planning through efforts of the Vermont
Ethics Network, are available to assist individuals
in exploring alternatives and choices involved in writing
living wills and selecting durable powers of attorney
for health care. If you or members of your family would
like to arrange a meeting with one of them, please call
Martha Clark at (802) 257-8214. She will arrange to
have one of the members of the Advance Care Planning
Initiative help you with any of the following:

thinking through your healthcare choices for the future

forming your thoughts about living wills
-
choosing an agent as a durable power of attorney for
healthcare

talking with your family about your preferences

talking with your healthcare providers about your
preferences

learning about palliative care and what adequate pain
management means to you
