DISCHARGE PLANNING at BMH

Patients often need assistance planning for their discharge from the hospital. Issues such as choosing a nursing home or rehabilitation facility are among the services provided by the Discharge Planning Department at BMH. The staff can also provide advice on obtaining equipment that will allow the patient to recover safely at home.

Nancy and Lisa are registered nurses who can assist patients and family members in planning and coordinating the patient's care following their discharge from the hospital. They will also set up home health services for the patient should it be recommended. Call Nancy at (802) 257-8354 or Lisa at (802) 251-8428 for assistance with planning for a patient's discharge from BMH.

SOCIAL SERVICES at BMH

Our social worker is skilled in counseling and providing services to patients and their families who are experiencing emotional distress, mental health issues, and/or substance abuse. Karen Gore has a master's degree in social work from Smith School for Social Work, and has many years of experience. In addition to providing professional help to both inpatients and outpatients, Karen also assists with arrangements for continued care after discharge.
