PATIENTS - PLEASE NOTE:
Before going to the Lab on the Basement level, you must
stop by Patient Registration / Admitting (on the main
floor of the hospital. That office is the 1st door to
the left off the front lobby. Once registered, you may
proceed to the Lab one floor down.
HOURS OF OPERATION / TELEPHONE NUMBERS
The laboratory is staffed 24 hours a day, 7 days a week.
However we are fully staffed for outpatients and routine
testing Monday through Friday from 7:00am to 6:00 pm
and Saturday from 8:00 am to 12:00 noon.
Appointments are not necessary when coming in for laboratory
testing during routine hours. However, please remember
to stop by Patient Registration / Admitting to register
for your test.
If patients come during off-hours they may have longer
periods of waiting due to reduced staff, and the laboratory
should be notified in advance.

TESTING REQUIREMENTS
The State law requires that any laboratory testing performed
can only be done when ordered by a physician or allied
health professional. Patients should have the yellow
laboratory order form that the physician has filled
out with them when coming in for testing. Make sure
the order form has your correct information on it including:
a. patient name
b. date of birth, sex
c. physician (care-giver) signature
d. tests ordered checked off or listed under "other"
e. diagnosis(es) to support medical necessity of the tests
being ordered
f. patient demographic information including social security
number and insurance information if being seen for the
first time at BMH or if the information has changed When
coming to the hospital for the testing, you must first
stop at the Patient Registration/Admitting department,
which is to your left upon entering the hospital lobby.
After registration you should proceed down to the laboratory
by going down the stairway or elevator to the basement.
Go to the Laboratory window where the staff will take
care of you, process your orders and collect the specimens
appropriate for the tests ordered. For specific instructions

What happens to your test results?
All patient test results are protected and confidential.
The laboratory will give out results only to the ordering
physician or allied health care professional who has requested
the test. If it was noted on the order form to send the
results to another physician this will also be done. If
the patient wants to receive the results, they must sign
an "Authorization for release of information" form.
After the physicians receive the results, they
will follow-up with you. There are many factors that affect
the results, and your health professional will be best
able to interpret them for you. |
