Wendy Cornwell Appointed BMH Director of Community Health Initiatives

Brattleboro Memorial Hospital announced today that Wendy Cornwell, RN, BS, BSN has been appointed Director of Community Health Initiatives/Blueprint Project.

For the past 14 years, Cornwell has been the Director of Community Health and Hospital Education at BMH, a role in which she managed hospital education, education for regulatory requirements and community involvement. In her new role, Cornwell will serve as the Project Manager for the Vermont Blueprint for Health initiative, responsible for the hiring and supervision of the Community Health Team, and working with hospital-owned and independent primary care practices, including Grace Cottage Hospital Practices, to assist practices to successfully achieve certification as a Patient-Centered Medical Home under the National Committee for Quality Assurance (NCQA).

“As BMH continues to expand its commitment to Vermont’s Medical Home initiative and healthcare reform, Wendy’s focus is now strictly on community health initiatives and the medical home project,” says BMH President and CEO Steven R. Gordon.

The goal of the Community Health Team is to assist patients with chronic and pre-chronic health conditions, develop active plans to improve their ability to manage their health and conditions more effectively in order to achieve better health outcomes. It will be Cornwell’s responsibility to manage the reporting infrastructure, fiscal, and health information technology aspects required by the Department of Vermont Health Access and insurers to meet the goals of the Vermont Blueprint for Health in the Brattleboro hospital service area.

In addition, Cornwell will be managing ongoing community issues such as the Community Health Needs Assessment as required by the Affordable Care Act, the Wellness in Windham County Health Education Calendar, the annual Wellness in Windham Health Festival — which is a new collaborative effort with Grace Cottage Hospital and the Brattleboro Retreat — and the maintenance of active membership in community coalitions.

Cornwell’s new office will be on the ground floor of the Dunham Building where she will be joining the Community Health Team.
